Starting the journey toward Supported Living Services (SLS) can feel overwhelming—but it doesn’t have to be. At New Beginnings for Exceptional People, we believe that understanding the process is the first step toward building independence, confidence, and long-term success.

If you or your loved one are considering SLS, this guide will walk you through who is eligible, how to apply, and what to expect during the intake process, starting with Alta Regional Center.

Where Do You Start?

Before receiving Supported Living Services, individuals must first be connected to a Regional Center. In Northern California, Alta Regional Center is the entry point for accessing services for individuals with developmental disabilities.

Who Is Eligible for Regional Center Services?

To qualify for services through Alta Regional Center, an individual must have a developmental disability that:

  • Began before age 18
  • Is expected to continue indefinitely
  • Causes significant limitations in daily life

These limitations may affect:

  • Self-care
  • Communication
  • Learning
  • Mobility
  • Self-direction
  • Independent living

Common qualifying conditions include:

  • Autism spectrum disorder
  • Intellectual disability
  • Cerebral palsy
  • Epilepsy
  • Other related conditions

Eligibility is determined through an intake and assessment process, not just a diagnosis alone.

Understanding the Intake Process

Here’s what you can expect:

Step 1: Submit an Application

Start by contacting Alta Regional Center to request an intake appointment. This can typically be done online or by phone.

You’ll provide basic information about the individual applying for services.

Step 2: Gather and Submit Documentation

To determine eligibility, Alta may request:

  • Medical or diagnostic records
  • Psychological evaluations
  • School records (IEPs, assessments)
  • Any previous service documentation

If you don’t have everything ready, Alta will help guide you through what’s needed.

Step 3: Intake Meeting

An intake coordinator will meet with you to:

  • Learn more about the individual’s history and needs
  • Explain available services
  • Answer questions
  • Begin the evaluation process

This is an important opportunity to share your goals and ask questions.

Step 4: Assessment & Eligibility Decision

Alta will review all submitted information and may conduct additional assessments if needed.

A decision is typically made within a set timeframe (often up to 120 days, unless urgent circumstances apply).

You will receive a determination stating whether the individual is:

  • Eligible
  • Not eligible
  • Or if more information is required

What Happens After Eligibility Is Approved?

Once eligibility is confirmed, a Service Coordinator will be assigned and they will work to create an Individual Program Plan (IPP).

This plan outlines:

  • Personal goals
  • Areas of support
  • Recommended services

If independent living away from family is a goal, Supported Living Services (SLS) may be included as part of the plan.

When Do Supported Living Services Begin?

SLS is typically introduced when:

  • The individual wants to live more independently
  • The IPP team agrees it is an appropriate support
  • The individual is ready to build or strengthen independent living skills

At that point:

  • The Service Coordinator will refer you to 3 different SLS providers to ensure choice
  • After interviewing all providers you will select an SLS provider
  • An Individualized Support Plan (ISP) will be developed by the provider
  • If required, the SLS provider will work to help secure housing
  • Services begin based on the individual’s goals and pace
